You do not need a referral. Speech pathology is a service you can access directly. I have worked with adults in their homes in Arncliffe, Hurstville, Marrickville, Bankstown and Rockdale, as well as via telehealth across New South Wales.

Starting earlier gives the best chance of meaningful functional gains. The Stroke Foundation is clear that aphasia therapy is most effective when it begins as soon as a person is medically stable, but that gains are still achievable years after a stroke with the right approach.

After stroke or brain injury

Aphasia, dysarthria and cognitive-communication difficulties are three different problems that often show up together after a stroke. Aphasia affects word-finding, comprehension, reading and writing. Dysarthria affects how clearly you speak. Cognitive-communication affects memory, organisation, and following a conversation. I work on all three, and carer and family education is part of every aphasia programme, because the people around the person with aphasia are their communication partners in daily life.

Progressive neurological conditions

Parkinson's disease, motor neurone disease and multiple sclerosis change speech over time. Starting speech therapy before communication becomes significantly restricted builds strategies and reserves. I can also begin AAC planning early, before it becomes urgent.

Voice difficulties

Hoarseness that keeps coming back. Vocal fatigue at the end of the day. Loss of volume. Strain from heavy voice use. I work with teachers, health workers and others who use their voice professionally. For anyone with signs of structural vocal cord involvement, I arrange referral to an ENT specialist.

Stuttering

Adults who stutter and want to do something about it. The Camperdown Programme is the evidence-based approach for adult fluency. I also work on situational strategies for job interviews, phone calls and presentations, where fluency anxiety is highest. More on this at stuttering therapy Sydney.

Cognitive-communication after brain injury

Memory, attention, word-finding, and organising thoughts in conversation. Adults managing long COVID, traumatic brain injury, or returning to work after acquired brain injury often come to me with problems that no other part of their rehabilitation has addressed. Cognitive-communication is a gap in most rehab systems.

Swallowing and Home Care Packages

Speech pathology under a Home Care Package is most commonly used for swallowing disorders, voice difficulties, and communication support after stroke or the onset of a progressive condition. I work within the package budget and coordinate directly with the care coordinator. For significant swallowing disorders, dysphagia assessment and management is a clinical priority and can be funded through the package.

Home-based delivery is my standard approach for adults who cannot travel.

I work across the Inner West, St George, Canterbury-Bankstown and the Eastern Suburbs, visiting homes and community settings in Tempe, Hurstville, Marrickville, Rockdale, Bankstown, Kogarah and Bexley as part of my regular service area.

Home-based therapy for adults has strong evidence behind it, particularly for stroke rehabilitation and voice work. Practice happens in the actual environment where the person needs to communicate, and that transfer is the whole point. Sitting in a clinic chair practising tasks that will not arise until you are at home misses the most important step.

I speak English and Mandarin. For adults from Chinese-speaking backgrounds, sessions run in English, Mandarin, or both.

    One to two sessions of formal and informal assessment, then a written report with results, functional interpretation and recommended goals.

    Clear goals, session frequency and review points, written in plain English.

    Regular sessions

    Progress notes after every visit. Reviews every 10 to 12 sessions. Carers and family members are welcome and often recommended.

Do I need a GP referral to book?
No. You can book directly. A GP referral is only needed for Medicare CDM rebates. DVA and NDIS do not require one either.
Can therapy help years after a stroke?
Yes. Early intervention gets the fastest gains, but the brain retains neuroplasticity well beyond the acute recovery phase, and meaningful progress is achievable years later. I always recommend booking an assessment to find out where things stand.
Can a family member join sessions?
Yes. For aphasia and cognitive-communication work, having a carer or family member in the session is often recommended. Strategies they learn and use between sessions matter as much as what we do in the session itself.
Is telehealth as effective as in-person for adults?
For many presentations, yes. Voice therapy, fluency work and cognitive-communication goals translate well to telehealth. For early post-stroke motor speech work requiring close observation, in-person is usually better. We work out the best option on the discovery call.
Can a Home Care Package fund speech therapy?
Yes. Adults receiving a Home Care Package can fund speech pathology under the allied health and clinical care component of their package. I deliver sessions in the home, work within the package budget, and coordinate directly with the care coordinator. No GP referral is required to begin, and assessment, therapy and progress notes are all included.
Do you work with my other health providers?
Yes. I share progress reports with GPs, neurologists, geriatricians, physiotherapists and occupational therapists where consent is given. For adults post-stroke, rehabilitation is a team effort and speech pathology is one part of it. Referrals are accepted from GPs, neurologists, geriatricians, hospital discharge teams and allied health providers. Self-referral is equally welcome.
