Who HCP speech therapy is for
Home Care Packages are available to older people (generally 65 and over, or 50 and over for Aboriginal and Torres Strait Islander people) who have been assessed through My Aged Care and allocated a package at Level 1, 2, 3, or 4. Speech pathology under HCP is appropriate for a range of conditions:
Post-stroke aphasia, dysarthria, or cognitive-communication difficulties. Progressive neurological conditions like Parkinson’s disease or motor neurone disease, where speech and voice change over time. Voice difficulties linked to age-related changes, lung conditions, or medications. Communication changes associated with dementia, where strategies can support daily life and family interactions. Apraxia of speech where the motor planning of spoken words has been disrupted.
If you are unsure whether speech pathology is right for your HCP, talk to your care provider or your GP. HCP funds are allocated according to an individual care plan developed with your provider.
How HCP speech therapy works at Shine and Speak
HCP arrangements work through your approved aged care provider. Your provider allocates a portion of your package funds to speech pathology as part of your care plan. I invoice your provider directly at the rate agreed in the service agreement. You have no out-of-pocket cost beyond what your provider has arranged.
I coordinate directly with your HCP care manager to ensure the service agreement, session frequency, and clinical goals are aligned with your overall care plan. This avoids double-coordination for you and keeps reporting consistent.
Session structure follows standard speech pathology: initial assessment, therapy plan, regular sessions with progress notes, periodic goal reviews. Therapy duration is based on clinical need rather than a fixed session count. Progress reports are provided to your care manager as required, and I attend care plan review meetings by phone or video where the care manager wants speech pathology input. Where the goal is communication with family members or staff, I include the people who spend the most time with you in parts of the sessions so the strategies actually carry over into daily life rather than sitting inside a therapy hour.
For HCP recipients who are also on DVA, NDIS (rare but possible for people who entered NDIS before turning 65), or Medicare CDM, I can navigate the overlap with your care manager so you are not double-billed.
HCP speech therapy across Sydney
Home visits across the Inner West, St George, Canterbury-Bankstown, and Eastern Suburbs, based in Arncliffe. Regular visit suburbs include Tempe, Rockdale, Hurstville, Bankstown, Marrickville, Bexley, Kogarah, and Burwood. For HCP recipients living in aged-care-supported accommodation within my service area, I visit the facility. For HCP recipients living independently at home, sessions happen wherever you live.
Home-based delivery is almost always the right choice for HCP speech pathology. Older adults managing communication difficulties benefit strongly from practising in their own environment with familiar objects, routines, and family members around.
For Mandarin-speaking older adults, bilingual sessions are available. This is often important for post-stroke rehabilitation where recovering communication in the person’s first language is usually more effective.
