When to consider stuttering therapy

For preschoolers, parents usually call because the stutter has been going for a few months and is not settling down on its own. Some will. A lot of preschool stuttering resolves without any intervention if the child is young enough and the stutter is mild. But the ones that do not resolve early? The gap between catching it at three and catching it at seven is enormous in terms of how much therapy is needed and how well it works.

For older children and teenagers, the trigger is usually that stuttering is now affecting school participation or social confidence. A child who was coping in early primary school may find it harder as class presentations become more frequent and peer-group dynamics shift.

Adults are a different story. The trigger is usually something concrete. A job change. A promotion that now means presenting to the board. A career pivot where phone calls are non-optional. Some adults I see have been managing their stutter for twenty years and are honestly quite good at it, but they have hit a ceiling and want something different. Others come in because their existing toolkit is not keeping up with what their life currently demands.

The programmes I use

For preschoolers, the Lidcombe Programme is the gold standard and has been for decades. It was developed at the University of Sydney, not a minor credential, and the outcome evidence is strong across multiple independent studies. The thing that surprises some parents: Lidcombe is parent-delivered, not therapist-delivered. I train you on specific feedback patterns you use during everyday play and conversation at home. Weekly sessions with me check progress and fine-tune. It is a working partnership, not an hour a week where you hand your child over.

For older children, teenagers, and adults, the Camperdown Programme is the evidence-based approach. Camperdown is also Australian-developed, which means the protocols and outcome data come from clinics doing the same work I do, not from a very different health-system context. Camperdown teaches a specific speech pattern that the client applies in structured practice, then generalises into daily life through systematic practice in progressively harder situations. There are clear stages and the outcome metrics are measurable, so you know whether it is working.

Alongside the programme approaches, I work on avoidance and attitude. This is a piece that gets skipped in a lot of stuttering clinics. Many people who stutter have built up avoidance habits over years: avoiding specific words they know they will block on, avoiding situations where fluency matters (meetings, phone calls, presentations), sometimes avoiding whole career paths. The avoidance reduces quality of life more than the stutter itself does. Working on avoidance, alongside working on stuttering, is what turns programme outcomes into life outcomes.

Telehealth is available across Australia, and works well for adult stuttering therapy in particular. For preschoolers on the Lidcombe Programme, in-person sessions usually suit better because the parent coaching works best face-to-face.

For bilingual clients, stuttering therapy in English and Mandarin is available. The core principles of both programmes transfer across languages.

My preschooler has just started stuttering. Should I wait?
Not long. Some preschool stuttering resolves spontaneously in the first few months, but the ones that do not benefit hugely from early Lidcombe intervention. I usually recommend an assessment around six to twelve months after stuttering starts if it has not resolved.
Does the Lidcombe Programme use punishment?
No. Lidcombe uses parent verbal feedback that is overwhelmingly positive, with a small amount of neutral acknowledgement of stuttered moments. It is a positive-reinforcement-based programme and children generally enjoy it.
Will I ever be fully fluent?
For many preschoolers, yes. Lidcombe produces full or near-full fluency in the majority of cases. For adults, the honest answer is that while Camperdown can significantly reduce stuttering and increase control, complete fluency is not always the right goal or achievable outcome. Many adults prefer reliable control and reduced avoidance to chasing perfect fluency.
How long does the programme take?
Quite a while. Lidcombe Stage 1 is usually around 10 to 16 weeks of weekly sessions, and then Stage 2 maintenance runs for another 6 to 12 months but with visits tapering down to monthly. Camperdown for adults is typically 12 to 20 weekly sessions, then generalisation work that continues beyond that. Neither programme is a quick fix. The good news is that once the gains are consolidated, they tend to hold.
